If Your GLP-1 Medication Isn’t Delivering Results, Your Hormones May Be Why

GLP-1 receptor agonist medications, including semaglutide and tirzepatide, are producing real results for many patients. But among women in perimenopause and menopause, a consistent pattern keeps showing up in both clinical practice and published research. The medication works, but not as well as expected. Appetite suppresses. The scale moves. And then the visceral fat around the abdomen stays stubborn, energy never fully recovers, and results plateau well before the goal.

A 2026 Mayo Clinic-led study, published in The Lancet Obstetrics, Gynaecology, & Women’s Health, offered a significant part of the explanation. Postmenopausal women who were on both hormone therapy and tirzepatide lost about 35% more weight than women on tirzepatide alone. The hormone therapy group averaged 19.2% body weight reduction. Women not on hormone therapy averaged 14.0%.

Because this was an observational study rather than a randomized trial, the researchers were careful to note that they cannot confirm hormone therapy directly caused the additional weight loss. Women on hormone therapy may also have had better sleep, more energy, and greater consistency with diet and activity, all of which are downstream effects of hormonal stability.

That nuance does not weaken the implication. It expands it. If hormone therapy improves sleep, reduces cortisol-driven fat accumulation, restores insulin sensitivity, and increases the responsiveness of GLP-1 receptors in the brain, then every one of those pathways is a reason the combination outperforms GLP-1 alone. The mechanism explains the outcome, and the outcome matches what the mechanism would predict.
