In November 2025, the FDA announced the removal of the black box warning from hormone replacement therapy products, with the updated labeling taking effect in February 2026. That warning had shaped treatment decisions, and patient fear, for over twenty years. That change reflects what a growing body of evidence has been showing for years: the risks attributed to hormone therapy were applied to the wrong hormones, interpreted too broadly, and used to justify undertreating millions of women.
For the woman in perimenopause who is gaining weight she cannot explain, that context matters. The hormones involved in your weight gain are the same ones conventional medicine has been avoiding.
Perimenopause Weight Gain Is a Hormone Problem, Not a Willpower Problem
You are eating the way you always have. You may even be eating less. You are exercising. And you are still watching your waist expand in a way that feels entirely outside your control, because it is.
Perimenopause weight gain, especially the accumulation of visceral fat around the abdomen, is not caused by poor discipline. This is why the earliest perimenopausal symptoms are so often sleep-related: difficulty falling asleep, waking at 2 or 3 in the morning, a feeling of internal restlessness that no amount of sleep hygiene corrects. And yet progesterone is frequently the hormone that never gets tested.
The Sleep-Cortisol-Fat Connection
Here is where perimenopause weight gain stops being a mystery and becomes a mechanism.
Poor sleep, the kind that progesterone deficiency causes, dysregulates cortisol. Cortisol is your primary stress hormone. The WHI Study Scared Women Away From Hormones. Here’s What It Actually Tested.
The 2002 Women’s Health Initiative study generated decades of fear about hormone therapy and breast cancer. Many women today, and many of their physicians, are still making decisions based on assumptions drawn from that study.
Here is what the WHI actually used: synthetic progestins, specifically medroxyprogesterone acetate (Provera/MPA), combined with conjugated equine estrogens derived from pregnant horse urine. Neither of these is a bioidentical hormone. They do not share the molecular structure of the hormones your body produces. They do not behave the same way in your tissues.
Natural, bioidentical progesterone carries none of the risks associated with synthetic progestins. Research shows that bioidentical progesterone down-regulates estrogen receptors in breast tissue, actively protecting breast cells rather than stimulating them. The North American Menopause Society’s position statement on hormone therapy makes clear that risk profiles differ substantially between bioidentical and synthetic hormone formulations. The WHI findings are not transferable to a properly constructed bioidentical protocol. They never were.
The FDA’s February 2026 removal of the black box warning reflects the accumulating weight of that evidence. If you declined hormone therapy years ago based on the WHI, you deserve an honest reassessment of what that study actually showed, and what it didn’t.

Why am I gaining weight in perimenopause if my doctor says my hormone levels are normal?
Standard lab reference ranges are wide. A result that registers as “normal” can still be insufficient for you individually. More importantly, progesterone, the first hormone to decline in perimenopause, is frequently never tested. If your provider hasn’t checked progesterone specifically, ask for it, and ask to discuss your symptoms alongside the numbers rather than instead of them.
Does progesterone cause weight gain?
