Why a "normal" TSH can still leave you feeling hypothyroid
This is the part standard care skips, and it is the reason so many people are sent home still exhausted.
TSH is not a thyroid hormone. It is a pituitary signal, a message your brain sends to tell the thyroid to work harder. Reading only the TSH is like judging an engine by the gas pedal instead of the engine. It is one input, not the whole machine.
What actually drives your metabolism is T3, the active thyroid hormone. Most of what your thyroid releases is T4, a storage form that does very little until your body converts it into T3. Plenty of people do not convert T4 to T3 efficiently, because of stress, inflammation, nutrient gaps, age, or their own biology. When that conversion lags, your TSH and even your T4 can read inside the range while the active hormone your cells need runs low. The number on the page looks acceptable. You feel hypothyroid because, at the level that matters, you are.
This is also why so many people are still tired on levothyroxine. Levothyroxine is T4 only. It assumes your body will turn it into T3 on its own. For the people who do not convert well, more T4 does not solve the problem, and their dose gets adjusted up and down against a TSH that was never the right target.
We look at the full picture: TSH, Free T4, and Free T3, the active hormone most panels never measure. Then we treat the person, not the lab printout.
The symptoms we treat
An underperforming thyroid rarely shows up as one thing. It shows up as a slow, whole-body decline that is easy to blame on stress, age, or a busy life. The most common reasons people come to us:

How we treat the thyroid
When thyroid hormone is what you need, we use desiccated thyroid, also called natural desiccated thyroid or NDT. It is a bioidentical thyroid hormone that carries the full spectrum your own thyroid makes, T4 and T3 together, rather than T4 alone.
That difference is the whole point. T4-only therapy works for people who convert well. For the many who do not, giving the body direct T3 is often what finally resolves symptoms that more and more T4 never could. The TSH question, answered honestly
If you start thyroid therapy that includes T3 and later see your TSH drop below the reference range, you will probably be told that means your dose is too high. It often does not.
On full-spectrum thyroid therapy, a lower or suppressed TSH is frequently expected. Once you are taking thyroid hormone directly, your pituitary sends less of its own signal, so the TSH falls by design. What matters is whether your active hormone levels, Free T3 and Free T4, are in a healthy range and whether you feel well, not whether one upstream signal sits inside a range built for people on no therapy at all.
This is also why a physician should manage your thyroid, not a single lab number. Too little thyroid hormone leaves you symptomatic. Too much carries its own risks, including heart palpitations and effects on bone over time. The work is finding the level that resolves your symptoms without overshooting, and then keeping you there with monitoring. That is a judgment a reflexive look at a single TSH cannot make. My TSH is normal. Can I still have a thyroid problem?
Yes, and it is common. TSH is a pituitary signal, not a thyroid hormone, and it is only one input. What is desiccated thyroid, or NDT?
Desiccated thyroid, also called natural desiccated thyroid or NDT, is a bioidentical thyroid hormone that contains the full spectrum your own thyroid makes, T4 and T3 together. Standard levothyroxine contains T4 only and relies on your body to convert it to the active T3. For people who do not convert efficiently, giving direct T3 through NDT is often what resolves symptoms that T4 alone did not.
I am still tired on levothyroxine. Why?
Levothyroxine is T4 only. It works well for people whose bodies convert T4 into the active T3 efficiently. If you are not one of them, raising the T4 dose does not deliver more of the active hormone your cells actually need, and your TSH gets chased up and down while you stay symptomatic. We look at Free T3, not just TSH, and where it fits we use full-spectrum thyroid therapy that supplies T3 directly.
Why did my TSH drop below the range after starting therapy?
